This form MUST be signed by a parent appointing the temporary guardian prior to arrival at the park.

Credit Policy

Queensland Moto Park offers a Rider Credit of your choice for another day. Credits will only apply if you have contacted the QMP Office prior to sign on. Customer must have proof of purchase.

Situations resulting in a credit will include:

- Unsuitable weather conditions, resulting in unsafe riding conditions.
- "Special circumstances" include those circumstances that are beyond the person's control;

Some examples of "special circumstances" include:
• Medical reasons where a person's medical condition has changed to such an extent that he or she is unable to attend.
• Family/personal reasons such as death or severe medical problems within a family, or unforeseen family situations
• Mechanical problems, being vehicle or bike related before you enter QMP (any mechanicals problems occuring whilst at QMP are the owners responsibility)

A customer is NOT entitled to a credit if:
- If the customer has already made use of the Queensland Moto Park facilities
- Mechanical problems occur whilst at Queensland Moto Park (mechanical maintenance is the riders responsibility)
- If the customer is ejected from the park by QMP staff for any reason
